The HIVCare Programme, together with your Preferred General Practioner (PGP) will help you actively manage your condition. The programme gives you and your healthcare provider access to various tools to monitor and manage your condition and to ensure you get high-quality coordinated healthcare and the best outcomes.

Your PGP must be on the Premier Plus GP network to enroll you on the HIVCare Programme through HealthID provided you have given consent to do so. Members who are enrolled on the HIVCare Programme through a Preferred General Practioner who is on the Premier Plus GP Network will have access to one additional social worker consultation. Your healthcare provider needs to complete a Request for additional cover for Chronic Disease List conditions on the Chronic Illness Benefit and submit it for review
